The initials C.B.T. stand for Cock and Ball Torture, within Shibari this typically involves tying male genitalier with a 2-4mm thick rope.

'tying hands straight behind the back' This is the Japanise name for the tie more commonly refered to by English speakers as the strapardo in which the arms are brought together behind the back and bound.
